To access these services you will first need to register with Newcastle Medical Centre and receive a password from the Practice. You can either enquire at reception or alternatively download, complete and return the Online Access Consent Form below to the Practice, along with two forms of Identification (one photographic and one proof of address).

Once you have registered you will then be able to login to your Online Patient Access Account
