Registering with Newcastle Medical Centre couldn't be easier!
First, check that you reside within the Practice Catchment Area - if you do not, you can still register, however you will need to complete an out of area declaration form as some services will not be available. The form is available for download.
Then simply download the registration pack, complete the forms and send them to the practice email with a copy of your photographic ID and proof of address. If you are from an EEC country you must also provide a copy your EHIC card. There is also a leaflet attached for patients who are not "ordinarily resident" in the UK, as you should be aware that not every person is entitled to all their NHS care free of charge in England.
If you are a student, you can register with us via the Campus Doctor site. Please complete the online registration forms on the link opposite and provide a copy of your photographic identification within 7 days. Failure to provide the identification in this time scale means your registration will be invalid and you will need to complete the forms again.

Once registered we will give you an information pack, that will include details of summary care records.
National Data Opt Out
Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.
You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13. There is a link under resources or a page on the website called National Data Opt Out
